New Mexico Reentry Center (NMRC) is seeking an Intensive Case Manager to provide direct, wraparound support to justice-involved individuals and families navigating housing, recovery, employment, and stabilization services.
Role Overview
This role provides high-touch, relationship-driven case management for clients with complex needs. The Intensive Case Manager coordinates services, removes barriers, and ensures accountability while maintaining dignity, structure, and consistency.
Key Responsibilities
- Conduct client intake, assessments, and individualized service planning
- Provide ongoing case management, follow-ups, and documentation, and mentorship
- Coordinate housing, employment, healthcare, and recovery services
- Maintain accurate records, case notes, and compliance documentation
- Communicate with partners, staff, service providers, and agencies
- Support crisis intervention, referrals, and client advocacy
Qualifications
- Experience in case management, social services, housing, or behavioral health
- Strong communication, organization, and documentation skills
- Comfortable working with justice-involved or high-needs populations
- Ability to manage boundaries, accountability, and compassion simultaneously
- Bilingual (Spanish/English) preferred
